You have had a lot going on.

I would encourage you to make an appointment with your GP to get a new psychiatrist referral. Unless you are going through the public clinic, then they should just move you to a new doctor. 
As for NDIS sometimes you just have to be proactive and make sure the meeting for the review is scheduled. I have a support worker who helps me do this but if you don’t have a support worker you would need to do that. I suggest you call the NDIS sooner than later. They are tightening the budget so probably wont chase you
