@outlander - been thinking of you. Am sitting on the airport floor waiting to board my flight home and thought I better pop in. Caught up with the posts.
You are so worth it my dear friend. You care so much for people even when you have lots on your plate. Trust me to me you are.
For those jolts as you fall asleep I get them too. For me it is muscle spasms. Sometimes I wake because I dream am falling.
As for your ultrasound glad came out clear. Definitely insist they check out the pain. Your gp sounded unprofessional to just dismiss it!

Hello @outlander Feeling for you.
I don't know if this will help, but as far as I know, all of your senses are perceptional events, which to me means that your brain takes in everything around you and within you and perceives (figures out?) what is going on. It then sends chemicals back out into your body. I found this helped understand how a person might see something or hear something that wasn't there, or even visa versa. Stress is real, your brain perceives that and it causes real physical reactions. There may not be a lot your Doctors can do, though I imagine if your symtoms persist (and I think it's important to tell them each visit) they might look at other possibilities or try different medications. (Though I've never had any success with medications myself, except for sleep assist ones, which they won't give me and you may find they won't give to you, as those medications can sometimes be abused and even dangerous). You are the best hope for you feeling better, you may not be able to avoid the stress, but never give up trying to lessen it - I believe less stress will make you feel much better. Easier said than done, but keep trying, for your sake.
